Consent of the patient to the processing of his personal data

I hereby declare that I accept that all the personal data that I provide to the professionals of the biennaitre center will be integrated into the center's database and processed for the following purposes:

• To ensure quality health services in a systemic framework which must take into account a set of medical and personal data relevant to my treatment.

• To be able to comply with the legal obligations to which health professionals are subject, including relations with the CNS, the invoicing of services and within the framework of the patient's right to a patient file carefully kept up to date and kept in a safe place.

• To communicate to you any information relating to our services, news and events.

This data may be provided to the prescribing doctor and to any other healthcare professional subject to professional secrecy.

This data will only be provided to third parties after prior written consent from me.
